Vendor ID Request Must Include
- Vendor Name
- Number of cards being issued
- Access level to be assigned
- Duration of time card needs to be active
Access Request for Vendors and Private Contractors
Vendor Card Access Requests
- Vendor Card Access Requests are for third parties that need door access. Vendors must have an official ǧÃŬAV Sponsor. The request for a vendor card or renewed access must come from a ǧÃŬAV Department such as the ǧÃŬAV project manager or ǧÃŬAV event coordinator and come from their official ǧÃŬAV Email and not directly from the Vendor. The department will send an email to Card Services for any access requests. A vendor card form listing vendor ID numbers and the access with expiration dates will be signed at the time of pickup. All Vendor cards can only be set up with temporary access and must have an expiration date included in the request.

- Cards may be collected and can remain on file for future use in ǧÃŬAV Card Services at no additional charge.
Private Contractor Cards
- Private Contractor Cards is for vendors that have a permanent presence on campus, such as dining, the bookstore, and dorm management. ǧÃŬAV Auxiliary Services is contract administrator for these vendors. Auxiliary Services will first approve the scope of card usage and building hours after which they may choose to allow the Contractor to set designated access approvers from their own staff who may send requests for cards and access directly to ǧÃŬAV Card Services to receive ID card like ǧÃŬAV Employees.
- Request must first come from their Private Contractor Supervisor that has prior authorization from ǧÃŬAV Auxiliary to send request to Card Services
- New employee will come to card service with a state or federal ID for identification to get a ǧÃŬAV private contractor card.
- ǧÃŬAV Card Services will only make private contractor card for the staff that need card access to ǧÃŬAV buildings. ǧÃŬAV Card Services does not make ID card for private contractors if the sole purpose is identification.
Important
It is the responsibility of the department to report any lost or stolen vendor cards.
